To be considered for placement into the Targeted Sports Program (TSP) in 2027, complete the online application form below. Applications forms will open the year before intake.
TSP Application Forms
2027 applications must be received a week before the trial date. Trial dates are available on our Facebook page. Applications will not be accepted after this time.
To apply to the TSP program during regular trial periods, please select and complete the online form for the required sport.
To apply to the TSP program outside of regular trial periods, please complete the following Expression of Interest form.
Physiotherapy Consent Form
Parents/carers can use this form to provide consent for their contact details to be shared with our school’s elected physiotherapist. The physiotherapist may then contact you regarding follow-up care for an injury sustained by an Eagle Vale Sports High School student.
